If you are a provisional psychologist undertaking a supervised practice program that you started before 1 July 2010 please use the Forms for Supervision of 4+2 Intern Programs for reporting to the Board.
If you are a general psychologist undertaking a specialist title supervision program in Western Australia that you started before 18 October 2010 please use the Forms for WA Specialist Title Supervision Programs for reporting to the Board.

Renewal of registration
Provisional psychologists who have already renewed their registration twice since the beginning of the National Registration Scheme in 2010 are not permitted to renew registration a third time in accordance with section 64(3) of the National Law. Instead a new application must be made using the form Application for provisional registration after three years of provisional registration for psychologists – ARPT-76 which is available under Provisional registration forms below
