For programs commenced prior to 18 October 2010
The forms below are only for Psychologists completing a specialist title supervision program that was approved by the Psychologists Registration Board of Western Australia before the commencement of the National Scheme and which they were part way through completing on participation day (18 October 2010). The transition provisions for specialist title supervision programs will apply until 30 June 2013 therefore these forms will only be valid until 30 June 2013.
Written requests for an extension to continue specialist title supervision beyond 30 June 2013 due to special circumstances will be considered by the Regional Board (NT/SA/WA) of the Psychology Board of Australia.
Psychologists undertaking a registrar program leading to an area of practice endorsement that was approved by the Psychology Board of Australia after 1 July 2010 should only use the forms of the Psychology Board of Australia.
